Reporting Injuries
Work related injuries and/or diseases are to be reported directly to the managed care organization (MCO) by completing a First Report of Injury (FROI) form.
Ask the injured worker or employer who their MCO is or ask to see their MCO identification card. If this information is not known, the Provider may look up the MCO
on the BWCs website at www.ohiobwc.com and select Employer/MCO lookup or call BWC at 1-800-OHIOBWC and press 42.
By rules, providers must report an injury to the appropriate MCO within 24 hours or one business day of the initial treatment or initial visit. MCOs are under an
obligation to successfully transmit FROIs to BWC within 24 hours after receiving notice of an injury, therefore the MCO relies heavily on providers to supply important
medical information.
If the provider does not have a FROI form, notice may be supplied to the MCO if all the important medical data is present to identify and transmit the FROI to BWC.
The data elements providers should give to MCOs when reporting a new injury include if available:
Injured worker information
- Name, address
- Telephone number
- Date of birth
- Gender
- Marital status
- Social Security Number
- Occupation
Injury information
- Type of accident (accident, occupational disease, death)
- Date of injury (date of death if applicable)
- Accident description (detail account of how the accident happened)
- Date of initial treatment
- Date last worked
- Return to work date (estimated if exact date is unknown)
- Will injury likely result in more than 7 days off work? (yes or no)
- ICD-9 diagnosis codes (including primary ICD-9)
- ICD-9 location (right, left, bilateral)
- ICD-9 site (digits or teeth)
- Injury description (body part injured, e.g. first joint of left index finger)
- Is diagnosis causally related to this industrial accident?
- Employer Information (Where is the injured worker employed)
- Employer name
- Employer Address
- Employer telephone
Provider Information
- Initial treating provider name and BWC provider number
- Physician of record name and BWC provider number
